Sends a distress alert and your identity to the rescue services by satellite. An EPIRB transmits a distress signal on 406 MHz to satellites, identifying your vessel, and most now include GPS so the alert carries your position. It works far beyond VHF range. The device that shows up on a ship's radar is a SART.
